Mayo Clinic Health System believes that health is more than the absence of illness. It includes an environment that supports the physical, emotional and social well-being of residents. In fact, studies show that many of the factors that have the greatest impact on our health occur in our communities.
Mayo Clinic Health System is pleased to offer a Hometown Health Grant as part of our investment to improve the health of the communities we serve. The Hometown Health Grant supports innovative efforts to improve mental health, prevent obesity and reduce chronic disease in local schools, workplaces and neighborhoods. This grant works in partnership with community-based organizations and residents to translate their vision for healthy communities into visible, concrete changes — and, ultimately, a healthier hometown.
The first step in the Mayo Clinic Health System Hometown Health application process is the submission of a letter of intent. Only one letter of intent per organization is accepted.
The letter of intent should not exceed two pages and must include the following information:
Applicants whose letter of intent is selected will be invited to submit a full application. Those applicants also may be asked to submit additional documents as part of the application process.
Invitations to submit a full application will be emailed the week of March 20, 2017. Grant recipients will be notified via email or telephone beginning Monday, May 15, 2017.
